Not able to reactivate wireless service

I am trying to reactivate after a cancellation, followed the steps by the page to reactivate doesn't seem to be working.

    Do you still have the activate code of old sim? I was told you can use the old sim card if you have the activation code.
    Worst case you just pay $5 to get a new sim, it will work for sure.

    I was under the impression you should be able to reactivate? Best to contact support to make sure.
